Registered Nurse, Home Health

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Deliver expert nursing care to patients in their residences, which includes administering medications, performing wound care, and conducting comprehensive patient evaluations.

• Collaborate with physicians and other healthcare professionals to create and execute tailored care plans.

• Observe patients' conditions and communicate any changes promptly.

• Educate patients and their families on managing diseases, understanding medications, and exploring treatment options.

• Keep precise patient care records and liaise with healthcare professionals effectively.

• Update the patient's physician and Clinical Manager on the progress of patient care and conditions.

• Supervise Home Health Aides and ensure proper completion of documentation.

• Offer individualized one-on-one care to assist patients in regaining their independence at home.

⛳️ Requirements

• Experience in Med surg, ICU, ER, or acute care settings.

• Must possess a current and unrestricted Registered Nurse license.

• Current CPR certification is required.

• Proven experience in collaborating with a team of healthcare professionals.

• A valid driver's license is necessary.

• Must have auto insurance.

• Reliable transportation is required.

• One year of nursing experience is preferred.

• Previous experience in Home Health is preferred.

• Participation in TB screening is mandatory if selected.

• Must maintain personal vehicle liability insurance as per state minimum limits or specified Humana limits.
