Registered Nurse – Case Manager

This is a fully remote position, open to applicants in California, +1 more state.

📋 Description

• Oversee and ensure the delivery of high-quality, cost-efficient care for patients with ESRD and CKD.

• Reduce fragmentation within the healthcare delivery system.

• Operate independently while collaborating with the healthcare team.

• Facilitate care coordination among patients, families, healthcare providers, care teams, the community, and the Regional Operations Manager.

• Extend support to ESKD and CKD patients across California, Oregon, Nevada, and Washington as necessary.

• Travel within the designated territory.

• Adapt to patient requirements and nephrology partner availability within a Monday to Friday schedule.

⛳️ Requirements

• An A.D.N degree from an accredited nursing school is required; a three-year diploma from an accredited diploma program may be accepted in place of the nursing degree.

• A B.S.N is preferred.

• A current Registered Nurse (RN) license in the state of practice is essential, with licensure in California, Oregon, and Washington required or a commitment to obtain any missing licenses.

• Maintenance of continuing education credits as mandated by the state of practice is required.

• Experience in case management is necessary; alternatively, at least 3 years of experience in assessing patient needs, creating and managing tailored care plans, coordinating interdisciplinary services, and facilitating complex healthcare transitions.

• Current CPR certification is mandatory.

• Previous experience in healthcare performance coaching is essential.

• Proven knowledge of data and management regarding clinical, financial, and patient satisfaction outcomes.

• Demonstrated effectiveness as a change agent is required.

• Knowledge of Continuous Quality Improvement (CQI) techniques is needed.

• Capability to adapt to new electronic processes and heightened productivity expectations for Capella implementation.

• Must possess a current driver's license in the state of employment, a favorable driving record, and the ability to meet insurance coverage criteria.

• Basic computer skills and proficiency in MS Word and Outlook are required.

• Functional proficiency with DaVita clinical software programs, including Capella, is expected within 90 days of employment.

• A home office with internet connectivity of at least 1MB upload and 1MB download speed is required.

• A minimum of five years of clinical nursing experience is strongly preferred.

• At least three years of renal nursing experience is preferred.

• Certification as a Nephrology Nurse (CNN) or Certified Case Manager (CCM) is preferred.
