Registered Nurse – Care Manager

This is a fully remote position, open to applicants in Wisconsin.

📋 Description

• Assist individuals enrolled in the Wisconsin Family Care Program by providing care coordination, support, and educational resources.

• Evaluate the health and safety needs of members to create detailed Member Care Plans.

• Perform routine health assessments.

• Collaborate with acute and primary care service providers.

• Inform members about disease processes, associated risks, nutrition, exercise, and lifestyle changes.

• Maintain documentation in member files within the mandated DHS contract timelines.

• Engage in monthly phone communications with members.

• Travel to conduct in-person visits in members' environments.

• Work remotely from home, with approximately 40% travel required for member visits.

• Report directly to the Manager of Care Coaching.

⛳️ Requirements

• Associate degree in nursing.

• Active Registered Nurse license in Wisconsin (in good standing).

• Minimum of 1 year of experience in Care Management (preferred).

• Must reside within 45 minutes or less of the designated coverage area (preferred).

• Previous experience in Care Management (preferred).

• Valid driver's license for the state.

• Proof of personal vehicle liability insurance with minimum coverage of 100/300/100.

• Completion of TB screening is mandatory if selected.

• Access to home internet with a download speed of at least 25 Mbps and an upload speed of 10 Mbps.

• A dedicated workspace free from interruptions to safeguard member PHI / HIPAA information.
