Registered Nurse, Bilingual – Vietnamese, English

This is a fully remote position, open to applicants in Virginia.

📋 Description

• Conduct Medicare Annual Wellness Visits through telehealth under the supervision of a physician and in accordance with CMS guidelines.

• Virtually assess and update patient medical histories, medication lists, immunization records, and family backgrounds.

• Evaluate current health status, vital signs when accessible, cognitive functions, and chronic disease risk factors utilizing remote monitoring tools where applicable.

• Offer virtual patient education regarding preventive measures, lifestyle modifications, and community resources.

• Create and revise personalized prevention plans tailored to patient requirements and physician recommendations.

• Collaborate with the supervising physician on documentation and follow-up activities using electronic health record systems.

• Identify HCC codes that require recapture reporting to CMS.

• Ensure accurate patient records are maintained in compliance with HIPAA and organizational protocols.

• Facilitate virtual referrals to healthcare services and specialists.

• Monitor patient progress and adherence to prevention plans through telehealth follow-ups, offering support and modifications as necessary.

• Keep abreast of Medicare guidelines, telehealth best practices, preventive care, and chronic disease management.

• Engage in virtual team meetings, training sessions, and continuing education.

⛳️ Requirements

• Fully bilingual in Vietnamese and English.

• Residing in GA.

• Current and unrestricted Registered Nurse (RN) license in the state of GA.

• Availability for PRN work from Monday to Friday, 8:00 am to 5:00 pm, for approximately 20 hours per week.

• A minimum of 3 years of nursing experience in patient care.

• Experience with Medicare Annual Wellness Visits (AWVs) and telehealth platforms is highly desirable.

• Strong understanding of Medicare guidelines, telehealth practices, and preventive care strategies is preferred.

• Excellent interpersonal and communication abilities.

• Proficient in electronic health records (EHR) systems, telehealth technology, and remote medical documentation.

• Ability to work independently and manage time efficiently in a remote work environment.

• Strong organizational skills and meticulous attention to detail.

• Internet connection with a minimum of 100 Mbps download and 10 Mbps upload speed.
