Registered Nurse, Bilingual (Vietnamese, English)

This is a fully remote position, open to applicants in United States.

📋 Description

• Perform Medicare Annual Wellness Visits through telehealth in alignment with CMS guidelines.

• Conduct virtual reviews and updates of patient medical histories, medication lists, immunization records, and family histories.

• Evaluate current health status, vital signs when accessible, cognitive function, and chronic disease risk factors utilizing remote monitoring tools as needed.

• Offer virtual patient education regarding preventive measures, lifestyle modifications, and community resources.

• Create and revise personalized prevention plans tailored to patient needs and physician recommendations.

• Collaborate with the supervising physician on documentation and follow-up regarding identified health issues using EHR systems.

• Recognize HCC codes that require recapture reporting to CMS.

• Ensure comprehensive and accurate patient records are maintained in accordance with HIPAA and organizational standards.

• Coordinate virtual referrals to healthcare services and specialists.

• Track patient progress and adherence to prevention plans through telehealth follow-up sessions.

• Keep updated on Medicare guidelines, telehealth best practices, preventive care, and chronic disease management.

• Engage in virtual team meetings, training, and continuing education opportunities.

⛳️ Requirements

• Must be fully bilingual in Vietnamese and English.

• Reside in Georgia.

• Hold a current and unrestricted Registered Nurse (RN) license in Georgia.

• Availability for PRN work Monday through Friday, from 8:00 am to 5:00 pm, totaling approximately 20 hours per week.

• Minimum of 3 years of experience in patient care nursing.

• Experience with Medicare Annual Wellness Visits (AWVs) and telehealth platforms is highly desirable.

• Preferred knowledge of Medicare guidelines, telehealth practices, and preventive care strategies.

• Demonstrate excellent interpersonal and communication abilities.

• Proficient in electronic health records (EHR) systems, telehealth technology, and remote medical documentation.

• Capable of working independently and managing time effectively in a remote work environment.

• Strong organizational skills and meticulous attention to detail.

• Reliable internet connection suitable for video calls and internal/external systems, with a minimum of 100 Mbps download and 10 Mbps upload speeds.

• Must adhere to security policies and procedures to protect PHI, PII, and Guidehealth intellectual property.
