Registered Nurse, Bilingual – Vietnamese, English

This is a fully remote position, open to applicants in Texas.

📋 Description

• Conduct Medicare Annual Wellness Visits through telehealth under the direct supervision of a physician and in alignment with CMS guidelines.

• Review and refresh patient medical histories, medication lists, immunization records, and family histories virtually.

• Evaluate health status, vital signs when accessible, cognitive function, and chronic disease risk factors utilizing remote monitoring tools when applicable.

• Offer virtual patient education on preventive strategies, lifestyle modifications, and community resources.

• Create and revise tailored prevention plans based on patient requirements and physician recommendations.

• Collaborate with the supervising physician on documentation and follow-up for identified health issues utilizing EHR systems.

• Identify HCC codes that require recapture reporting to CMS.

• Ensure accurate patient records are maintained in accordance with HIPAA and organizational standards.

• Facilitate virtual referrals to healthcare services and specialists.

• Monitor patient advancement and adherence to prevention plans through telehealth follow-ups, providing support and adjustments as necessary.

• Stay informed about Medicare guidelines, telehealth best practices, preventive care, and chronic disease management.

• Engage in virtual team meetings, training, and continuing education.

⛳️ Requirements

• Fully bilingual in Vietnamese and English.

• Must reside in Georgia.

• Current and unrestricted Registered Nurse (RN) license in Georgia.

• Availability for PRN work Monday through Friday, from 8:00 am to 5:00 pm, approximately 20 hours per week.

• Minimum of 3 years of nursing experience in patient care.

• Experience with Medicare Annual Wellness Visits (AWVs) and telehealth platforms is highly desirable.

• Familiarity with Medicare guidelines, telehealth practices, and preventive care strategies is preferred.

• Excellent interpersonal and communication skills.

• Proficient in electronic health records (EHR) systems, telehealth technology, and remote medical documentation.

• Ability to work independently and manage time efficiently in a remote environment.

• Strong organizational skills and attention to detail.

• Internet connection with a minimum of 100 Mbps download and 10 Mbps upload speeds.

• Ability to remain accessible during specified working hours and participate in virtual meetings with the camera on.
