Registered Nurse, Bilingual – Vietnamese, English

This is a fully remote position, open to applicants in Texas.

📋 Description

• Perform Medicare Annual Wellness Visits through telehealth, under the direct supervision of a physician and in line with CMS guidelines.

• Carry out necessary patient assessments and screenings.

• Conduct virtual reviews and updates of medical histories, medication lists, immunization records, and family histories.

• Evaluate current health status, cognitive function, and vital signs when available, along with chronic disease risk factors utilizing remote monitoring tools as appropriate.

• Provide education to patients regarding preventive measures, lifestyle modifications, and community resources during virtual consultations.

• Create and revise personalized prevention plans tailored to patient needs and physician recommendations.

• Collaborate with the supervising physician on documentation and follow-up for any identified health issues.

• Recognize HCC codes that necessitate recapture reporting to CMS.

• Ensure the maintenance of accurate patient records in compliance with HIPAA and organizational policies.

• Facilitate virtual referrals to healthcare services and specialists.

• Track patient progress and adherence to prevention plans, offering support and modifications through telehealth follow-ups.

• Stay updated on Medicare guidelines, telehealth best practices, preventive care, and chronic disease management.

• Engage in virtual team meetings, training sessions, and continuing education opportunities.

• Safeguard patient and company data by adhering to security policies and protocols.

⛳️ Requirements

• Fully bilingual in Vietnamese and English.

• Must reside in Georgia.

• Current and unrestricted Registered Nurse (RN) license in the state of Georgia.

• Availability for PRN work Monday through Friday, from 8:00 am to 5:00 pm, totaling approximately 20 hours per week.

• A minimum of 3 years of nursing experience in patient care.

• Preferred experience in primary care, preventive care, or telehealth.

• Highly desirable experience with Medicare Annual Wellness Visits (AWVs) and telehealth platforms.

• Knowledge of Medicare guidelines, telehealth practices, and preventive care strategies is preferred.

• Exceptional interpersonal and communication abilities.

• Proficiency in electronic health records (EHR) systems, telehealth technology, and remote medical documentation.

• Capacity to work independently and manage time effectively in a remote environment.

• Strong organizational skills and meticulous attention to detail.

• Reliable internet connection suitable for video calls and internal/external systems.

• Minimum internet speed of 100 Mbps for download and 10 Mbps for upload.

• Must adhere to security policies and procedures that protect PHI, PII, and company intellectual property.

• Must be available during the specified working hours.

• Must participate in virtual meetings with cameras enabled.

• Required to disclose any secondary employment.
