Registered Nurse, Bilingual (Vietnamese, English)

This is a fully remote position, open to applicants in Texas.

📋 Description

• Perform Medicare Annual Wellness Visits via telehealth in accordance with CMS guidelines.

• Review and revise patient medical histories, medication lists, immunization records, and family histories.

• Evaluate health status, cognitive function, and vital signs when available, as well as chronic disease risk factors.

• Utilize remote monitoring tools when relevant.

• Educate patients on preventive strategies, lifestyle adjustments, and community resources.

• Create and revise individualized prevention plans.

• Work collaboratively with supervising physicians on documentation and follow-up tasks.

• Identify HCC codes that necessitate recapture reporting to CMS.

• Ensure accurate patient records are maintained in adherence to HIPAA and organizational standards.

• Coordinate virtual referrals to healthcare services and specialists.

• Track patient progress and compliance with prevention plans through telehealth follow-ups.

• Keep up-to-date with Medicare guidelines, telehealth best practices, preventive care, and chronic disease management.

• Engage in virtual team meetings, training sessions, and continuing education.

⛳️ Requirements

• Fully bilingual in Vietnamese and English.

• Must reside in Georgia (GA).

• Current and unrestricted Registered Nurse (RN) license in the state of Georgia (GA).

• Availability for PRN work Monday through Friday, from 8:00 am to 5:00 pm, totaling approximately 20 hours per week.

• A minimum of 3 years of experience in patient care nursing.

• Experience with Medicare Annual Wellness Visits (AWVs) and familiarity with telehealth platforms is highly desirable.

• Preferred knowledge of Medicare guidelines, telehealth practices, and preventive care strategies.

• Exceptional interpersonal and communication abilities.

• Proficient in electronic health records (EHR) systems, telehealth technology, and remote medical documentation.

• Capability to work independently and manage time efficiently in a remote environment.

• Strong organizational skills and meticulous attention to detail.

• Internet connection with minimum speeds of 100 Mbps download and 10 Mbps upload.
