Registered Nurse, Bilingual – Vietnamese, English

This is a fully remote position, open to applicants in Texas.

📋 Description

• Conduct Medicare Annual Wellness Visits through telehealth, ensuring direct supervision by a physician and adherence to CMS guidelines.

• Review and refresh patient medical histories, medication lists, immunization records, and family histories.

• Evaluate health status, assess vital signs when available, cognitive function, and chronic disease risk factors utilizing remote monitoring tools as needed.

• Offer virtual education on preventive measures, lifestyle modifications, and available community resources.

• Create and revise personalized prevention plans with guidance from a physician.

• Collaborate with the supervising physician on documentation and follow-up processes using EHR systems.

• Recognize HCC codes that require recapture reporting to CMS.

• Ensure accurate patient records are maintained in compliance with HIPAA and organizational standards.

• Facilitate virtual referrals to healthcare services and specialists.

• Track patient progress and adherence to prevention plans through telehealth follow-ups.

• Stay abreast of Medicare guidelines, telehealth best practices, preventive care, and chronic disease management.

• Engage in virtual team meetings, training sessions, and ongoing education.

⛳️ Requirements

• Must be fully bilingual in Vietnamese and English.

• Candidate must reside in Georgia.

• Hold a current and unrestricted Registered Nurse (RN) license in Georgia.

• Available for PRN work Monday through Friday, from 8:00 am to 5:00 pm, totaling approximately 20 hours per week.

• A minimum of 3 years of experience in patient care nursing is required.

• Experience with Medicare Annual Wellness Visits (AWVs) and telehealth platforms is highly desirable.

• Familiarity with Medicare guidelines, telehealth practices, and strategies for preventive care is preferred.

• Possess excellent interpersonal and communication skills.

• Demonstrate proficiency in electronic health records (EHR) systems, telehealth technology, and remote medical documentation.

• Ability to work independently and effectively manage time in a remote environment.

• Strong organizational abilities and attention to detail are essential.

• Employees must provide their own internet connection with minimum speeds of 100 Mbps download and 10 Mbps upload.

• Must adhere to security policies that protect PHI, PII, and Guidehealth intellectual property.
