Registered Nurse, Bilingual, Spanish – English

This is a fully remote position, open to applicants in United States.

📋 Description

• Conduct Medicare Annual Wellness Visits through telehealth under the direct supervision of a physician.

• Complete necessary CMS assessments and screenings.

• Review and update patient medical histories, medication lists, immunization records, and family histories via virtual consultations.

• Assess current health status, cognitive function, vital signs when available, and chronic disease risk factors utilizing remote monitoring tools as applicable.

• Provide virtual patient education on preventive measures, lifestyle modifications, and community resources.

• Develop and revise personalized prevention plans tailored to patient needs and physician recommendations.

• Collaborate with the supervising physician on documentation and follow-up regarding identified health issues using EHR systems.

• Identify HCC codes that necessitate recapture reporting to CMS.

• Maintain precise patient records in accordance with HIPAA and organizational standards.

• Facilitate virtual referrals to healthcare services and specialists.

• Monitor patient progress and adherence to prevention plans through telehealth follow-ups.

• Stay updated on Medicare guidelines, telehealth best practices, preventive care, and chronic disease management.

• Participate in virtual team meetings, training sessions, and continuing education opportunities.

⛳️ Requirements

• Fully bilingual in Spanish and English.

• Must reside in Georgia.

• Current and unrestricted Registered Nurse (RN) license in Georgia.

• Availability for PRN work Monday through Friday, from 8:00am to 5:00pm, totaling approximately 20 hours per week.

• Minimum of 3 years of nursing experience in patient care.

• Experience in primary care, preventive care, or telehealth is preferred.

• Experience with Medicare Annual Wellness Visits (AWVs) and telehealth platforms is highly desirable.

• Knowledge of Medicare guidelines, telehealth practices, and preventive care strategies is preferred.

• Proficient in electronic health records (EHR) systems, telehealth technology, and remote medical documentation.

• Ability to work independently and manage time efficiently in a remote environment.

• Strong organizational skills and meticulous attention to detail.

• An internet connection with a minimum of 100 Mbps download and 10 Mbps upload speeds.

• Ability to adhere to security policies and procedures that protect PHI, PII, and company intellectual property.

• Willingness to disclose any secondary employment.

• Availability during the specified working hours.

• Willingness to engage in virtual meetings with cameras on.
