Registered Nurse, Bilingual – Spanish, English

This is a fully remote position, open to applicants in United States.

📋 Description

• Perform Medicare Annual Wellness Visits via telehealth in accordance with CMS regulations.

• Review and revise patient medical histories, medication lists, immunization records, and family histories.

• Evaluate health status, vital signs when accessible, cognitive function, and chronic-disease risk factors utilizing remote monitoring tools.

• Educate patients on preventive strategies, lifestyle modifications, and community resources through virtual consultations.

• Create and update personalized prevention plans under the direction of a physician.

• Collaborate with the supervising physician on documentation and follow-ups utilizing EHR systems.

• Identify HCC codes that necessitate recapture reporting to CMS.

• Ensure accurate patient records are maintained in compliance with HIPAA and organizational standards.

• Assist in facilitating virtual referrals to healthcare services and specialists.

• Track patient progress and adherence to prevention plans through telehealth follow-ups.

• Remain informed about Medicare guidelines, telehealth best practices, preventive care, and chronic disease management.

• Engage in virtual team meetings, training sessions, and continuing education opportunities.

⛳️ Requirements

• Must be fully bilingual in Spanish and English.

• Must reside in Georgia.

• Hold a current and unrestricted Registered Nurse (RN) license in Georgia.

• Availability for PRN work from Monday to Friday, 8:00 am to 5:00 pm, approximately 20 hours per week.

• At least 3 years of nursing experience in patient care.

• Experience with Medicare Annual Wellness Visits (AWVs) and telehealth platforms is highly desirable.

• Preferred knowledge of Medicare guidelines, telehealth practices, and preventive care strategies.

• Excellent interpersonal and communication abilities.

• Proficient in electronic health records (EHR) systems, telehealth technology, and remote medical documentation.

• Capability to work independently and manage time efficiently in a remote environment.

• Strong organizational skills and keen attention to detail.

• Minimum internet speed of 100 Mbps download and 10 Mbps upload.

• Ability to adhere to security policies that protect PHI, PII, and company intellectual property.

• Availability during designated working hours and participation in virtual meetings with the camera on.

• Must disclose any secondary employment.
