This is a fully remote position, open to applicants in Idaho.
• Organize, plan, and oversee home health and hospice services.
• Conduct initial, comprehensive, and ongoing assessments for patients and their families.
• Deliver physical assessments, nursing care, medications, and treatments in accordance with the physician's care plan.
• Create and update personalized care plans, setting goals for patients.
• Initiate preventive and rehabilitative nursing interventions.
• Educate and counsel patients and families regarding healthcare, medications, diet, self-care, and related needs.
• Assist patients with daily living activities, promoting self-sufficiency and comfort care.
• Serve as a case manager, coordinating care for a designated caseload.
• Maintain precise clinical documentation and record pain and symptom management.
• Communicate effectively with physicians, community health professionals, patients, families, and caregivers.
• Collaborate with the interdisciplinary team and participate in team meetings.
• Engage in on-call responsibilities as needed.
• Ensure that necessary equipment, services, and supplies are readily available.
• Supervise support staff and delegate tasks when appropriate.
• Take part in continuing education, quality assessment, and performance enhancement initiatives.
• Graduate from a nursing program accredited by the National League for Nursing.
• Minimum of two years of nursing experience, with at least one year in public health, home care, or hospice nursing preferred.
• Registered nurse with a current license to practice professional nursing in the state.
• Exhibits strong observation, problem-solving, verbal, and written communication skills; nursing competencies as per the checklist.
• Capable of independently organizing and prioritizing workload.
• Must be able to lift, position, and/or transfer patients.
• Must be able to lift supplies and equipment.
• Requires visual acuity and hearing to perform necessary nursing tasks.
• Must hold a current CPR certification.
• Must possess and maintain a valid driver's license, have automobile insurance, and access to a vehicle.
