This is a fully remote position, open to applicants in Washington.
• Lead a team of IKC Registered Nurses responsible for case managing dialysis and Chronic Kidney Disease (CKD) patients.
• Oversee the operational implementation of innovative care models for CKD Medicare Advantage patients.
• Supervise quality and performance metrics, including STARs metrics.
• Facilitate RN recruitment and oversee the onboarding process.
• Provide clinical leadership and training for RNs.
• Act as a pivotal leader in clinical and operational strategy alongside senior and cross-functional leadership.
• Manage Disease Management programs within the specified market or geographic area.
• Analyze and review data to enhance patient care outcomes.
• Develop and maintain relationships with both internal and external stakeholders.
• Offer clinical support to Registered Nurse Case Managers and provide direct nursing care when necessary.
• Lead direct reports and handle employment and HR functions, including payroll, PTO, hiring, onboarding, professional development, performance management, and disciplinary actions.
• Ensure adherence to regulations and DaVita policies.
• Monitor compliance with risk management and quality assurance standards.
• Achieve quality and metric objectives in Integrated Kidney Care programs.
• Coordinate activities with DaVita IKC management, Regional Medical Directors, cross-functional departments, and nursing staff.
• Evaluate processes for cost-effectiveness and work to minimize operational expenses.
• Implement initiatives and assess their impact on outcomes.
• Prepare and submit reports and analyses regarding team performance.
• Travel within the designated market and occasionally outside of it.
• A minimum of 5 years of management experience leading and overseeing teams is required.
• Experience in managing data-driven outcomes and process improvements is essential.
• Background in Primary Care and value-based care is required.
• Exceptional communication skills regarding medical management, operational, and business-related matters.
• Preferred minimum of 5 years in clinical nursing leadership.
• Valid Registered Nurse (RN) licensure is required.
• Intermediate computer skills are necessary.
• Proficiency in MS Word, Excel, PowerPoint, and Outlook is required.
• A home office with an Internet connection of at least 1MB upload and 1MB download speed is mandatory.
• Candidates must reside in the Mountain or Central time zone.
• Willingness to travel as needed is required.
• An unencumbered Registered Nurse license is essential.
