This is a fully remote position, open to applicants in United States.

📋 Description

• Work alongside healthcare providers to support them in achieving optimal performance with value-based contracts.

• Partner with provider staff to create and implement enhancements in operational and clinical processes.

• Identify, develop, and set key performance metrics for providers associated with value-based contracts.

• Pinpoint opportunities for improvement based on key performance indicators and formulate action plans.

• Create and deliver training as well as support to provider staff regarding performance metrics and operational and clinical enhancements.

• Foster collaboration among provider groups to exchange best practices and opportunities for performance improvement.

• Address system, process, or resource issues by responding to provider feedback and concerns.

• Cultivate collaborative relationships with both internal and external customers to enhance process improvements and clinical outcomes.

• Engage in meetings with providers, the VBC team, and the internal PHCO clinical team.

• Participate in Joint Operating Committees alongside VBC providers, the VBC team, and the internal PHCO clinical team.

• Lead and facilitate clinical/quality discussions representing the internal Care Management team.

• Collaborate with the corporate team to refine and enhance departmental processes and procedures.

• Carry out additional responsibilities as assigned.

• Adhere to all policies and standards.


⛳️ Requirements

• Must possess authorization to work in the U.S. without requiring employment-based visa sponsorship now or in the future.

• A Bachelor's Degree in Business Administration, Finance, Public Health, or a related field is mandatory.

• A minimum of 3 years of experience in value-based contracting, managed care, or process and performance improvement is required.

• A Master's Degree is preferred.

• Ability to work collaboratively with healthcare providers and their staff.

• Some travel may be necessary a few times per year.
