This is a fully remote position, open to applicants in Arizona, +1 more state.
β’ Directly oversee a multidisciplinary team consisting of Nurse Practitioners, Registered Nurses, Care Navigators, and LVNs.
β’ Recruit, onboard, train, mentor, manage performance, implement corrective actions, and dismiss staff when necessary.
β’ Conduct team meetings, individual check-ins, and performance assessments.
β’ Manage daily operations of field-based care programs across medical, behavioral, and social sectors.
β’ Ensure that contractual SLAs and KPIs are achieved or surpassed.
β’ Establish performance dashboards and quality assurance measures.
β’ Develop and implement a palliative care program for members facing advanced or end-of-life conditions.
β’ Collaborate with senior leadership to align program objectives, allocate resources, and execute strategies for performance, quality, and outcomes.
β’ Engage in health plan-led patient rounds for complex cases.
β’ Provide clinical leadership and consultation for intricate case management and care planning.
β’ Direct evidence-based protocols, motivational interviewing techniques, trauma-informed care, and risk stratification methodologies.
β’ Facilitate transitions of care and ensure appropriate service referrals.
β’ Maintain HIPAA compliance, uphold patient data confidentiality, ensure audit readiness, and implement corrective measures.
β’ Spearhead quality improvement initiatives across care coordination.
β’ Serve as the primary point of contact for payers, providers, and community partners.
β’ Represent APG at health-related events, coalitions, advisory boards, and business development engagements.
β’ Collaborate with public health agencies and community organizations to enhance service access.
β’ Perform additional duties as assigned by management.
β’ A minimum of four (4) years of nursing experience, ideally in population health, care coordination, or community-based programs.
β’ At least two (2) years of progressive leadership experience in population health, care coordination, or community-based settings.
β’ Proven success in recruiting, training, supervising, and managing the performance of multidisciplinary teams.
β’ Extensive knowledge of SLAs, KPIs, Medi-Cal and Medicare Advantage programs, and palliative care operations.
β’ Strong understanding and application of HIPAA and patient privacy regulations.
β’ Exceptional analytical, problem-solving, communication, and interpersonal abilities.
β’ Proficient in EMR platforms (Salesforce preferred) and Microsoft Office Suite.
β’ Willingness and capability to travel frequently both in-state and out-of-state as necessary.
β’ BSN is required.
β’ Active and unrestricted Nurse Practitioner (NP) license.
β’ Certification in Case Management (CCM) and Palliative Care (e.g., ACHPN) to be obtained within two years of position commencement.
β’ Current BLS certification is mandatory.
β’ A valid and clear driverβs license is required.
β’ Proof of automobile insurance is necessary.
β’ Ability to physically perform essential duties, including lifting 30 lbs., transporting materials, stooping, kneeling, crouching, reaching, balancing, walking, standing, talking, hearing, and typing.
