Professional – Community Based

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Deliver community-focused care and case management for the Community Support Team (CST) in Robeson County.

• Guarantee that clients receive exceptional care in their homes or community settings.

• Coordinate care efforts and advocate on behalf of clients.

• Update personal care plans (PCPs) and input documentation into the system.

• Engage in the development of PCPs.

• Ensure the quality of care and safety for the individuals served.

• Support individuals and families through various facilities, community environments, or secure virtual care options.

• Collaborate with an interdisciplinary team to foster holistic wellness and recovery.

⛳️ Requirements

• Must fulfill the criteria to be a NC Qualified Mental Health Professional.

• A minimum of a bachelor’s degree is required.

• At least 2 years of experience working with the served population.

• Possession of a current, valid NC driver’s license.

• Insurance coverage with a clean driving record.

• Successful completion of a comprehensive background check.

• Successful motor vehicle records evaluation.

• Successful drug screening required.

• Verification of required educational credentials.

• Licensure or certification credentialing with SEIC is necessary.
