Practice Transformation Manager, Bilingual Spanish

This is a fully remote position, open to applicants in California.

📋 Description

• Spearhead the implementation and rollout of value-based care models, which encompass population health management, practice transformation, care coordination, and risk-based contracting.

• Work in partnership with providers and practice personnel to enhance clinical and administrative workflows by utilizing evidence-based guidelines, care pathways, and quality improvement frameworks.

• Implement and refine the use of technology solutions within practices, including EHRs, patient portals, and care management tools.

• Ensure adherence to all pertinent regulations and accreditations concerning value-based care.

• Create and execute population health management programs tailored to the needs of high-risk patient populations, aiming to boost patient engagement.

• Collaborate with practices to pinpoint and execute strategies that fulfill quality performance objectives, adapting methods based on the unique requirements of the practice and patient population.

• Facilitate care coordination among healthcare providers, patients, and families to guarantee a smooth care delivery process.

• Evaluate and manage the risks associated with value-based care contracts, including risk stratification and mitigation strategies.

• Develop and implement systems for performance measurement to assess the effectiveness of VBC interventions and ensure practice accountability.

• Leverage all available data sources to uncover improvement opportunities, assess outcomes, and support decision-making processes.

• Cultivate robust, trust-based relationships with practice administrators, providers, and staff to foster collaboration and sustainable transformation.

• Regularly collaborate with Medical Directors, Regional VPs, and internal Vytalize teams to align on practice priorities and exchange best practices.

⛳️ Requirements

• Bachelor’s degree in Healthcare Administration, Public Health, Nursing, or a related discipline.

• Over 3 years of experience in practice transformation, care coordination, population health, or a similar healthcare operations role.

• Direct experience with physician practices or provider organizations within a value-based care framework.

• Strong comprehension of value-based care models and primary care practice operations.

• Capacity to manage multiple practice relationships concurrently while delivering quantifiable results.

• Proficient in working with data and performance metrics to steer practice discussions and interventions.

• Bilingual; Spanish proficiency is preferred.

• Clinical background or direct experience in a primary care or ambulatory care environment is preferred.

• Familiarity with EMR systems, HEDIS measures, and population health management platforms is preferred.
