Bilingual Care Coordinator, Full-Time

This is a fully remote position, open to applicants in United States.

📋 Description

• Conduct monthly phone consultations with the designated practice.

• Make 30 to 50 outbound calls each day.

• Complete necessary patient interactions to achieve CCM service objectives.

• Educate patients regarding chronic illnesses and their care plans.

• Coordinate care with healthcare providers, pharmacies, and community resources.

• Identify care gaps and social determinants affecting health.

• Develop and update tailored care plans within the EMR system.

• Accurately record all patient interactions.

• Ensure compliance with HIPAA regulations and maintain patient confidentiality.

• Adhere to a fixed schedule from Monday to Saturday, 10:00 AM to 6:30 PM in the designated time zone.

• Spend 7 to 8 hours daily engaging on the phone.

• Meet monitored productivity and quality standards.

⛳️ Requirements

• A minimum of 1 to 3 years of experience as a CMA/RMA or LPN.

• Active certification or multistate licensure for LPNs.

• Excellent verbal and written communication skills in English.

• Comfortable being on the phone for the majority of the day.

• Familiarity with EMR/EHR systems.

• Organized, dependable, and capable of working independently in a remote environment.

• A private, quiet home office free from interruptions.

• Reliable high-speed internet connection (100 Mbps or higher).

• A second monitor is mandatory.

• Experience with geriatric patients is preferred.

• Background in care coordination or case management is preferred.

• Proficient verbal and written communication skills in Spanish are preferred.
