This is a fully remote position, open to applicants in Australia.
• Provide evidence-based physiotherapy assessments, interventions, and exercise prescriptions for individuals living with heart failure.
• Carry out virtual assessments using phone and video platforms.
• Employ digital health technologies for exercise prescription, monitoring, and clinical support.
• Track patient outcomes and clinical risks, escalating concerns and collaborating with the wider care team.
• Enable patient-centered goals that promote health, wellbeing, and self-management.
• Work collaboratively with cardiologists, nurses, care coordinators, hospital teams, and primary care providers.
• Assist in early discharge initiatives and minimize preventable hospital admissions.
• Spearhead physiotherapy service development and contribute to the Hearts at Home care model.
• Engage in quality improvement, service evaluation, and program development activities.
• Offer expert guidance, mentoring, and supervision to colleagues.
• Ensure the maintenance of clinical documentation, activity reporting, and regulatory compliance.
• Advocate for the Hearts at Home program and facilitate stakeholder engagement.
• Participate in orientation, education, and professional development activities.
• Current registration as a Physiotherapist with AHPRA.
• At least five years of post-registration experience, including a minimum of three years in cardiac and/or cardiopulmonary rehabilitation settings.
• Highly developed skills in clinical assessment, rehabilitation, and exercise prescription within cardiac and/or heart failure environments.
• Experience in telehealth or virtual care assessment and rehabilitation is highly regarded.
• Strong clinical reasoning and evidence-based decision-making skills.
• Expertise in interpreting medical management, diagnostic investigations, treatment plans, medications, and biometric observations.
• Excellent communication skills and stakeholder engagement abilities.
• Capability to work both autonomously and effectively within multidisciplinary teams.
• Experience in leading service improvement, quality initiatives, or program development.
• Background in supervising, mentoring, or supporting fellow clinicians.
• Strong organizational, documentation, and time-management skills.
• Commitment to patient-centered care and continuous quality improvement.
