This is a fully remote position, open to applicants in California.

📋 Description

• Assess patients’ functional abilities, mobility, muscle strength, gait, and range of motion to create tailored treatment plans.

• Provide physical therapy services in alignment with the physician’s care plan and California’s Physical Therapy Practice Act.

• Instruct patients and their families or caregivers on therapeutic exercises, equipment usage, and recovery objectives.

• Work collaboratively with physicians and the healthcare team—reporting any changes in patient status and engaging in case conferences.

• Supervise Physical Therapy Assistants and oversee Home Health Aide care on a bi-weekly basis when physical therapy is the sole skilled service.

• Handle documentation including clinical progress reports and discharge planning.

• Recognize the need for additional home health services and facilitate referrals as necessary.

• Execute other responsibilities as required for patient care.

⛳️ Requirements

• Bachelor’s or Master’s degree in Physical Therapy from an accredited institution.

• Valid California Physical Therapy license.

• At least 2 years of experience in physical therapy; experience in home health is advantageous.

• Excellent communication skills with the ability to engage effectively with patients and their families.

• Current CPR certification from the American Heart Association.

• Valid driver’s license along with a reliable and insured vehicle.
