This is a fully remote position, open to applicants in California.
• Assess patients’ functional abilities, mobility, muscle strength, gait, and range of motion (ROM) to create personalized treatment plans.
• Provide physical therapy services according to the physician’s care plan and in compliance with California’s Physical Therapy Practice Act.
• Instruct patients and their families or caregivers on therapeutic exercises, equipment usage, and recovery objectives.
• Collaborate with physicians and the healthcare team—reporting any changes in patient conditions and participating in case discussions.
• Supervise Physical Therapy Assistants and, when physical therapy is the only skilled service, oversee Home Health Aide care on a bi-weekly basis.
• Handle documentation including clinical progress reports and discharge planning.
• Recognize the need for additional home health services and coordinate referrals as necessary.
• Perform other required duties related to patient services.
• Bachelor's or Master's degree in Physical Therapy from an accredited institution.
• Current California Physical Therapy license.
• At least 2 years of experience in physical therapy; background in home health is advantageous.
• Excellent communication skills and the ability to engage effectively with patients and their families.
• Up-to-date CPR certification from AHA.
• Valid driver's license and access to a reliable, insured vehicle.
