Pharmacy Care Coordinator – Clinical Pharmacy Operations

This is a fully remote position, open to applicants in Alabama, +3 more states.

📋 Description

• Assist clinical outreach initiatives by conducting outbound calls and utilizing portal chat to schedule pharmacist appointments addressing clinical gaps or medication issues.

• Facilitate cost-of-care efforts by contacting providers and members to propose lower-cost alternative medication options.

• Manage a designated caseload that requires follow-up with providers, pharmacies, and/or members regarding medication recommendations, concerns, or inquiries.

• Independently oversee assigned cases with prompt follow-up, clear next steps, and comprehensive documentation.

• Aid members in navigating their Devoted Health benefits.

• Engage in sprints, pilots, and special projects.

• Collaborate with the wider pharmacy team to achieve shared goals and address coverage needs.

• Assist in training and mentoring new team members.

• Participate in team and pharmacy department activities and responsibilities.

• Ensure clinical recommendations are communicated to external providers.

• Follow up with pharmacies regarding claims issues, pick-up confirmations, or medication discontinuations.

• Keep members updated on any changes to their medications.

⛳️ Requirements

• Minimum of 3 years of experience in health care or retail pharmacy.

• Current National PTCB Certification.

• Comfortable making a high volume of outbound calls to members, providers, and pharmacies.

• Capable of managing daily responsibilities and tasks autonomously.

• Ability to frequently shift between tasks, projects, and priorities.

• Highly organized and detail-oriented.

• Eager to learn new software and adapt as tools and workflows develop.

• Adaptable and flexible, with a positive attitude towards taking on new or changing tasks.

• A true team player who contributes innovative ideas, collaborates on solutions, and supports teammates.

• Comfortable communicating with physicians, pharmacies, and patients, demonstrating a genuine passion for our members.
