Pharmacist, Home Infusion

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Interpret and assess physician medication orders.

• Prepare, calculate, and dispense medications with precision.

• Identify and report any suspected adverse drug reactions.

• Support formulary management and therapeutic interchange protocols.

• Review and maintain patient medication records, noting allergies, drug interactions, and therapeutic duplications.

• Organize and assist in the daily operations of the pharmacy.

• Provide oversight and guidance to pharmacy technicians, aides, interns, and other staff.

• Ensure compliance with regulatory requirements, professional pharmacy standards, and hospital quality management policies.

• Communicate effectively with physicians, nurses, and care teams to clarify medication orders and assist in care decisions.

• Offer drug information, education, and support to healthcare professionals and pharmacy students.

• Utilize pharmacy information systems and clinical databases.

• Process and verify prescription orders efficiently.

⛳️ Requirements

• Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD) from an accredited institution.

• Valid Pennsylvania pharmacist license.

• Preferred: 3+ months of experience in a hospital pharmacy setting.

• Preferred: Experience or familiarity with home infusion and IV admixture preparation.

• Reliable home internet connection, if working remotely, via cable broadband or fiber optic service with a minimum of 10Mbps download and 5Mbps upload speeds.

• Ability to participate in rotating shifts, including evenings, weekends, on-call duties, and holidays.

• Compliance with the Pennsylvania Pharmacy Practice Act, Federal Drug Laws, and other regulatory standards.

• Adherence to state and federal prescription requirements.
