Pharmacist 2, Inpatient – Oncology

This is a fully remote position, open to applicants in New York.

📋 Description

• Approve medication orders for patients receiving treatment at the medical center.

• Oversee and actively engage in the compounding, preparation, and dispensing of medications while adhering to state and federal pharmacy laws and regulations.

• Supervise pharmacy technicians and manage their workflow effectively.

• Dispense medications with precision and in a timely manner.

• Address and resolve issues related to problematic medication orders.

• Process and verify medication orders utilizing information-system technology, ensuring the maintenance of accurate patient profiles.

• Review products that require pharmacist approval prior to dispensing.

• Participate in quality assurance activities, including medication incident reporting, MAR reconciliation, and adverse event/error reporting.

• Collaborate with physicians, nurses, healthcare professionals, and patients in the development or participation of clinical practice as part of an interdisciplinary team.

• Engage in focused clinical initiatives and conduct Medication Use Evaluations.

• Monitor the clinical progress of assigned patients, including renal/hepatic function, culture results, and laboratory tests.

• Suggest therapy modifications and document clinical activities and interventions accordingly.

• Provide education to Pharmacy staff and healthcare professionals.

• Offer patient education as necessary.

• Resolve patient care and operational challenges through effective communication and reasonable attempts at resolution.

• Take part in interdisciplinary teams, revising policies, procedures, or treatment guidelines when necessary.

• Fulfill charge pharmacist responsibilities, which include coordinating shift resources, addressing problems and staff conflicts, and facilitating communication between shifts.

• Reach out to supervisors when issues surpass individual expertise or authority.

• Carry out other assigned duties as required.

⛳️ Requirements

• A Bachelor's degree in Pharmacy is required.

• At least 1 year of relevant acute care pharmacy experience or successful completion of an accredited pharmacy practice residency is required.

• PharmD is preferred.

• An equivalent combination of education and experience will be considered.

• A valid Pharmacist license upon hire is required.

• Capability to compound, prepare, dispense, and validate medications in accordance with applicable pharmacy laws and regulations.

• Ability to supervise pharmacy technicians effectively.

• Proficiency in utilizing current information-system technology for medication order processing and patient profiles.

• Competence in monitoring renal/hepatic function, culture results, and other laboratory tests.

• Ability to recommend adjustments to medication therapy.

• Proficient in documenting clinical activities and interventions.

• Capable of participating in interdisciplinary clinical practice and medication-use evaluations.
