This is a fully remote position, open to applicants in United Kingdom.
• Process incoming referrals across various services and verify eligibility criteria.
• Review referral details and escalate any ineligible or complex cases to the MDT.
• Oversee and manage the NHS inbox, addressing GP inquiries and requesting further information.
• Follow up on missed assessment appointments according to protocol.
• Engage with service users via phone, email, and SMS throughout their journey.
• Follow up on pending forms, documentation, and incomplete tasks.
• Handle queries regarding appointments, group allocation, attendance, and DNAs.
• Process discharges in accordance with service protocols, including DNAs and non-engagement cases.
• Generate and dispatch discharge confirmation letters to service users.
• Prepare and send discharge correspondence to GP practices using NHS systems.
• Offboard service users from relevant systems and accurately close patient records.
• Maintain precise records across referral trackers and internal systems.
• Document patient activities in alignment with governance and audit standards.
• Assist with ongoing service administration and workflow management across teams.
• Collaborate with Patient Coordinators and clinical teams within established protocols, escalation pathways, confidentiality, and data protection regulations.
• Strong administrative and organizational abilities, capable of managing competing priorities in a fast-paced environment.
• Excellent written and verbal communication skills, comfortable addressing sensitive topics with patients and professionals.
• High attention to detail and accuracy in handling data, records, and correspondence.
• Ability to manage confidential information appropriately, adhering to GDPR requirements.
• Proficient in using digital systems and databases.
• Capable of identifying risks, obstacles, or errors and promptly escalating them to the appropriate parties.
• Experience in a healthcare, NHS, or patient-facing administrative role (desirable).
• Familiarity with referral pathways and healthcare processes (desirable).
• Experience working with service users or clinical teams (desirable).
• Fixed-term contract lasting 6 months.
• 40 hours per week.
• Core service hours are Monday to Friday, with occasional evening and weekend shifts as part of the rota.
