Patient Care Representative

This is a fully remote position, open to applicants in Texas.

📋 Description

• Perform daily phone outreach to former patients within the designated territory to assess their health status and identify any changes in condition that may require additional services.

• Facilitate connections between patients and the relevant internal team members based on their needs and requests, which may include skilled care, personal care, hospice care, and ElaraConnect personnel.

• Respond to inquiries about enterprise services and deliver the necessary information to patients.

• Collaborate with Patient Care Specialists to identify leads within the assigned market area.

• Work alongside Patient Care Specialists and service line operators to address any issues related to Check-In operations.

• Stay informed about the latest trends and innovations in virtual customer relations.

• Assist in assessing agency operations and systems to pinpoint opportunities for improving workflow and productivity.

• Engage in clinical record audits, utilization reviews, Peer Review, and Quality Improvement initiatives.

• Participate in training sessions, educational seminars, or other professional development opportunities.

• Alert the supervisor to any issues that require administrative intervention and attention.

• Accurately input data into company systems as required.

• Report directly to the Program Supervisor.

⛳️ Requirements

• High School Diploma or GED.

• Over 1 year of experience in a home health care environment is preferred.

• Familiarity with Medical Terminology.

• Proficient in MS Office Suite (Word, Excel, Outlook).

• Outstanding verbal and written communication abilities.

• Availability to work weekends and/or holidays as needed.
