This is a fully remote position, open to applicants in Kentucky, +1 more state.
• Serve as the patient’s advocate to ensure adherence to prescribed medication therapies and pharmacy care plans.
• Provide polite, professional, and high-quality service.
• Coordinate pharmacy care with healthcare professionals, payers, and other health-related entities.
• Address and resolve patient inquiries within designated time frames.
• Assist patients with issues related to drug therapy and pharmacy care.
• Encourage medication compliance.
• Report any adverse events associated with drug therapy and pharmacy care.
• Foster customer relationships with physician practices and enhance communication among patients, payers, pharmacists, nurses, labs, and other healthcare stakeholders.
• Engage in the administration of patients' pharmacy care plans.
• Troubleshoot and resolve issues faced by patients and payers.
• Support fellow healthcare team members in a collaborative environment.
• Identify and communicate opportunities for continuous quality improvement.
• Keep updated notes and documentation related to patients' drug therapy and pharmacy care plans.
• Coordinate responses and resolutions with relevant internal and external parties.
• Participate in case discussions with pharmacists, nursing services, pharmacy support managers, and other pharmacy care coordinators.
• Carry out related duties as assigned.
• Must reside in either Texas or Kentucky.
• High school diploma is required.
• A minimum of two years of experience in a healthcare-related call center, physician's office, or social service environment.
• Preferred educational background in business administration, accountancy, sales, marketing, computer sciences, or similar fields through a two-year associate degree program or an equivalent combination of experience and education.
• Strong time management and organizational abilities.
