This is a fully remote position, open to applicants in Minnesota.
β’ Conduct Comprehensive Assessments and administer the Comprehensive Assessment Survey.
β’ Perform GAIN-SS evaluations for all clients upon their admission.
β’ Create Initial Services Plans for each client.
β’ Reach out to referents, with appropriate consent, within the first three days of admission and continue weekly to provide updates on progress and consultation for ongoing care planning.
β’ Evaluate clients for potential vulnerabilities using the criteria outlined in the Program Abuse Prevention Plan.
β’ Formulate, review, and revise Individual Abuse Prevention Plans as necessary.
β’ Design comprehensive individualized treatment plans that include specific and measurable goals, objectives, and interventions within five sessions.
β’ Update and adjust treatment plans with clients in accordance with the timeline specified in 245G.
β’ Document client engagement and progress in client records following Center policies and procedures.
β’ Maintain regular communication with the Outpatient Clinic Director and Mental Health Professional regarding client treatment and progress.
β’ Lead group sessions focused on chemical dependency counseling and education.
β’ Create and implement individualized continuing care plans, which involve coordination with significant others and referrals for medical, substance use, mental health, and additional services.
β’ Provide support to concerned individuals, including family and friends, with resources for sober support during and after treatment.
β’ Engage in professional development related to Substance Use Disorders and Co-Occurring Disorders.
β’ Contribute to the broader Center team, including general housekeeping, kitchen duties, and light maintenance tasks.
β’ Undertake other duties as assigned.
β’ Current Full or Temporary licensure by the State of Minnesota as an Alcohol and Drug Counselor in good standing.
β’ Familiarity with the implications of chapter 245G, chapter 245A, and sections 626.556, 626.557, and 626.5572.
β’ Ability to operate a computer and other office equipment, including calculators, copiers, printers, etc.
β’ Frequent communication with employees, managers, patients, and other individuals with key relationships.
β’ Capability to accurately receive and convey information in these interactions.
β’ Regular movement around the office to access file cabinets, office machinery, supplies, etc.
β’ Must be a resident of Minnesota.
β’ Availability for full-time hours, 40 hours per week.
β’ Group sessions are held Monday through Thursday from 5 PM to 8 PM.
