Nurse Practitioner – Weekend On-Call

This is a fully remote position, open to applicants in Oklahoma.

📋 Description

• Deliver tailored, coordinated healthcare services to plan members residing in long-term care facilities.

• Enhance patient outcomes, improve quality of life, and minimize unnecessary hospital admissions.

• Manage patients with both acute and chronic conditions using independent judgment, consulting with a supervising physician as necessary.

• Collaborate effectively with providers, RN case managers, facility partners, and health plan leadership.

• Execute TruHealth’s national model of care while adhering to federal and state regulations.

• Evaluate patients’ medical and mental health requirements.

• Create comprehensive care plans that proactively address conditions and overcome barriers to care.

• Ensure adherence to standards set by local, state, and federal agencies regarding clinical services.

• Prescribe medications and initiate lab work, diagnostic procedures, and consultations.

• Oversee treatment adherence and patient responses, adjusting treatment plans as necessary.

• Offer training and guidance to facility staff when needed.

⛳️ Requirements

• Advanced practice provider: nurse practitioner (NP) or physician assistant (PA).

• Active FNP, AGNP, AHACNP or PA license is mandatory.

• Completed degree from an accredited APRN or PA program.

• Master’s degree or higher is required.

• Minimum of 3 years’ experience in clinical nursing or rehabilitation with geriatric populations.

• Background in primary care or internal medicine, preferably with a focus on geriatrics.

• Proficient in Electronic Health Records.

• Familiarity with Microsoft applications, including Word, Outlook, and Excel.

• Capacity to exercise independent judgment in treating patients with acute and chronic conditions.

• Commitment to compliance with all federal and state regulations.

• Experience in long-term care is preferred.
