This is a fully remote position, open to applicants in Rhode Island.
• Conduct In-Home Health Evaluations lasting approximately 45 minutes at the residences of members.
• Interact directly with members to address their health inquiries.
• Assess medications and review medical history.
• Carry out physical evaluations.
• Perform additional diagnostic tests as needed, including diabetic eye exams and spirometry.
• Utilize an iPad and clinical workflow for every evaluation.
• Establish trusted relationships to enhance health outcomes.
• Travel nationwide to visit members and connect them with appropriate healthcare resources.
• Engage in local travel and within licensed states as necessary.
• Conduct virtual visits when required.
• Deliver Signify Health Diagnostic & Preventive Services, such as spirometry, diabetic eye exams, and bone density screenings.
• Operate primarily in the private residences of members.
• Possess an active, unrestricted license(s) in the relevant coverage area(s).
• Board certification is required.
• Multi-state licenses are preferred.
• Capability to cover Rhode Island.
• Willingness to travel locally and within licensed states as necessary.
• Ability to safely perform tasks in homes that may have household pets, secondhand smoke, and airborne allergens or irritants.
• Qualification as a Nurse Practitioner or Physician Assistant.
