Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in Minnesota.

📋 Description

• Conduct in-home health evaluations lasting approximately 45 minutes.

• Build trusted relationships while engaging with health plan members in their residences.

• Review patients' medications and medical histories.

• Perform physical assessments.

• Execute additional diagnostic tests as requested by the health plan, such as diabetic eye exams and spirometry.

• Utilize an iPad and clinical workflow for each evaluation.

• Travel nationwide to connect individuals with suitable care.

• Travel locally and within licensed states as necessary.

• Conduct virtual visits when required.

• Deliver Signify Health services that include diagnostic and preventive services like spirometry, diabetic eye exams, and bone density screenings.

• Primarily work in the private residences of health plan members.

⛳️ Requirements

• Possess an active, unrestricted license(s) in the relevant coverage area(s).

• Board certification is required.

• Multi-state licenses are preferred.

• Qualification as a Nurse Practitioner or Physician Assistant.

• Willingness to travel locally and within licensed states as needed.

• Capability to conduct visits in homes that may have household pets, secondhand smoke, and other potential airborne allergens or irritants.

• Proficiency in using an iPad and navigating an intuitive clinical workflow.

• Expected weekly hours: 12.
