Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in Minnesota.

📋 Description

• Deliver In-Home Health Evaluations at the residences of members.

• Conduct visits lasting approximately 45 minutes.

• Engage directly with members to address health-related inquiries.

• Review medications and the medical background of members.

• Carry out physical assessments.

• Execute additional diagnostic tests as prescribed by the health plan, including diabetic eye examinations and spirometry.

• Utilize an iPad along with an intuitive clinical workflow for each evaluation.

• Travel nationwide to connect individuals with the necessary care.

• Travel locally and within licensed states as required.

• Conduct virtual consultations when necessary.

• Offer Signify Health services, encompassing Diagnostic & Preventive Services products.

⛳️ Requirements

• Possession of an active, unrestricted license(s) in the relevant coverage area(s).

• Board certification is mandatory.

• Multi-state licenses are preferred.

• Willingness to travel locally and within licensed states, as required.

• Capability to conduct virtual visits as necessary.

• Ability to safely perform tasks in homes that may have household pets, secondhand smoke, and other potential airborne allergens or irritants.

• Qualification as a Nurse Practitioner or Physician Assistant.
