Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in Connecticut.

📋 Description

• Conduct in-home health assessments at the residences of members.

• Perform visits lasting approximately 45 minutes each.

• Engage with members in a personal setting to address their health inquiries.

• Review patients' medication and medical history.

• Carry out physical evaluations.

• Administer additional diagnostic tests as prescribed by the health plan, such as diabetic eye exams, spirometry, and bone density screening.

• Utilize an iPad and follow clinical workflows for each assessment.

• Travel nationwide to connect individuals with the appropriate care.

• Conduct virtual visits as required.

• Offer Signify Health services, including products related to Diagnostic & Preventive Services.

• Establish trustworthy relationships to enhance health outcomes.

⛳️ Requirements

• Possession of an active, unrestricted license(s) in the relevant coverage area(s).

• Board certification is mandatory.

• Multi-state licenses are preferred.

• Capability to perform duties comfortably and safely in homes that may have household pets, secondhand smoke, or other potential airborne allergens or irritants.

• Willingness to travel locally and within licensed states as necessary.

• Qualification as a Nurse Practitioner or Physician Assistant.
