
Nurse Practitioner – National After-Hours Team, Part Time

This is a fully remote position, open to applicants in New York, +1 more state.
• Act as the primary support for residents and facility staff during after-hours, offering guidance and medical assistance via phone.
• Utilize Curana’s telephonic platform to manage calls, facilitating care coordination among facilities, hospitals, and clinics.
• Provide care for acute, chronic, and behavioral health conditions in partnership with physicians and specialty providers.
• Conduct a minimum of 30% telehealth consultations, which include evaluations for acute changes, falls, and controlled substance visits.
• Execute comprehensive assessments and accurately document encounters in the EMR, adhering to CMS requirements.
• Implement clinical protocols and practice guidelines to promote treatment in place whenever feasible.
• Engage in mandatory education and training sessions.
• Offer coverage on weeknights, weekends, and holidays for skilled nursing and senior living facilities.
• Master's Degree as a Nurse Practitioner.
• Current unrestricted NP license in the state of residence and the ability to obtain licenses in other necessary locations within 60 days.
• National Nurse Practitioner certification as ANP, FNP, or GNP.
• Capability to secure DEA licensure / Prescriptive Authority.
• Experience in managing acute and chronic diseases.
• Clinical background in adult, family, or geriatrics.
• At least 3 years of experience as a Nurse Practitioner.
• Ability to obtain a collaborative practice agreement, if required by state(s).
• Willingness to work scheduled shifts in line with scheduling policies.
• Proficient computer skills, including the ability to document medical information in both written and electronic medical records.
• Experience in a nursing home or with seniors in an acute care environment.
• Knowledge of Geriatrics, Chronic Illness, and acute disease management.
• Familiarity with Advanced Illness and end-of-life discussions.
• Skill in developing and maintaining positive customer relationships.
• Flexibility to adapt to change.
• Availability for overnight and holiday shifts; requirement of 2 holidays per year for part-time employees.
• Ability to conduct at least 30% of telehealth visits.
• Competence in managing high call volumes and multiple calls independently.
• Must be a resident of New York or Pennsylvania and capable of working across multiple states.
• Company is unable to offer visa sponsorship (H1B or otherwise).
• Part-time position with a fixed schedule of 24 hours per week.
