Nurse Case Manager – Patient Support Program

This is a fully remote position, open to applicants in Canada.

📋 Description

• Act as the main contact for patients participating in the Patient Support Program.

• Offer education about prescribed treatments, expectations, medication administration, and adherence strategies.

• Assist with patient enrollment and onboarding processes.

• Coordinate care and facilitate communication with physicians, clinic personnel, specialty pharmacies, and other healthcare professionals.

• Help patients navigate treatment access and reimbursement procedures.

• Perform patient assessments and follow-up actions as mandated by the program.

• Track patient progress and provide continuous support throughout their treatment journey.

• Ensure accurate and timely documentation within program systems.

• Identify and elevate clinical or operational issues as necessary.

• Participate in quality improvement initiatives and measures for program success.

• Develop and sustain positive working relationships with healthcare providers and clinic partners.

• Travel to clinics, healthcare facilities, and stakeholder meetings across Alberta and British Columbia.

⛳️ Requirements

• Possession of an active and unrestricted nursing license in Alberta and/or British Columbia (RN, RPN/LPN, or equivalent nursing designation).

• Diploma, degree, or a suitable combination of education and experience aligned with the P1 Professional Career Framework.

• Strong verbal and written communication abilities.

• Proficiency in Microsoft Office and electronic documentation systems.

• Capability to work approximately 2.5 days per week during program operating hours of 8:00 a.m. to 8:00 p.m. EST, from Monday to Friday.

• Willingness to travel to clinics and healthcare locations across Alberta and British Columbia as needed.

• Excellent organizational skills and meticulous attention to detail.

• A valid driver's license and access to dependable transportation may be essential based on territory coverage.

• Ability to operate independently in a remote setting while effectively managing competing priorities.

• Occasional lifting and transportation of educational or program materials may be necessary.
