This is a fully remote position, open to applicants in Canada.
• Offer assistance to patients and healthcare providers concerning Patient Support services, diseases, and therapies.
• Help patients and caregivers with evaluations related to Patient Support services.
• Act as a liaison among patients, families, and healthcare providers regarding medication accessibility.
• Respond to inquiries about program services, disease states, and products.
• Facilitate patient adherence through continuous disease- and product-related support.
• Refer patients to healthcare providers for clinical inquiries that exceed program materials.
• Record patient information and interactions in electronic health records.
• Perform regular file assessments and update records to accurately reflect patient journeys.
• Adhere to case-management protocols, which include data collection, follow-up, therapy disruption/discontinuation monitoring, adverse event reporting, and communication with primary healthcare providers.
• Collaborate with reimbursement specialists and physicians to secure therapy funding and investigate alternatives.
• Organize and monitor patient services in line with KPIs to hasten therapy access.
• Engage in specialized therapy training sessions and pharmaceutical meetings.
• Attend patient meetings and conferences, working alongside patient associations and advocacy groups as necessary.
• Coordinate with healthcare providers, pharmacies, and infusion/injection clinics on treatment logistics and problem resolution.
• Communicate with the manager, team, and client regarding patient case management.
• Assist in training new Nurse Case Managers and provide feedback on protocols, databases, processes, and interdepartmental workflows.
• Report adverse events and serious adverse events in accordance with program procedures.
• Diploma in healthcare or a related discipline.
• Licensed Nurse (RN or RPN/LPN) with active registration from the College of Nurses in the province of employment is essential.
• Capability to resolve issues and work both independently and collaboratively within a team.
• Strong analytical and problem-solving abilities.
• Ability to work efficiently across various functions and within a small team.
• Exceptional teamwork skills and readiness to provide constructive feedback.
• Ability to consistently produce high-quality professional work at a detailed level in a dynamic environment.
