Care Manager, RN

This is a fully remote position, open to applicants in United States.

📋 Description

• Oversee intricate care management for members primarily requiring physical health support

• Perform thorough assessments and analyze member needs, obstacles to care, and social determinants impacting health

• Create individualized, continuous care and service plans

• Coordinate care amongst members, families, caregivers, providers, specialists, and community resources

• Recognize issues and barriers to care, offering suitable interventions

• Monitor member condition, status changes, and progress towards care plan objectives

• Offer resource assistance for services including employment, housing, independent living, justice, and foster care

• Ensure prompt access to necessary care and services

• Conduct outreach via telephone, digital means, home visits, and/or other locations

• Collect, document, and maintain member data and care management activities to ensure regulatory compliance

• Educate members and their families/caregivers regarding disease processes, care gaps, provider instructions, care alternatives, referrals, and healthcare benefits

• Provide leadership insights on ways to enhance care quality and cost-effective service delivery

• Assist PHCO Case Management and work collaboratively with cross-functional Medicaid teams

• Adhere to all policies and standards while performing other assigned tasks

⛳️ Requirements

• Required credential: Registered Nurse (RN)

• Degree from an Accredited School of Nursing or a Bachelor’s degree in Nursing

• 2–4 years of relevant experience

• RN (Registered Nurse) state licensure and/or compact state licensure required

• Availability to work from 8:00 a.m.–5:00 p.m. Eastern Time

• Candidates must be residents of Georgia
