Nurse Case Manager

This is a fully remote position, open to applicants in Canada.

📋 Description

• Strategically plan and oversee intricate care coordination cases and activities.

• Ensure timely access for patients, facilitate treatment progression, and uphold continuity of care.

• Perform assessments and assist in care planning efforts.

• Manage transitions of care by addressing clinical, social, and access-related barriers.

• Collaborate with providers, pharmacies, payers, and internal stakeholders.

• Address access barriers, align subsequent actions, and ensure continuity within cases.

• Analyze case trends, recurring challenges, and workflow patterns to pinpoint root causes.

• Suggest enhancements to processes.

• Create and improve job aids, care coordination templates, and documentation protocols.

• Mentor less experienced team members in handling complex cases, escalation procedures, and documentation requirements.

⛳️ Requirements

• A bachelor's degree in nursing, social work, health sciences, public health, healthcare administration, or a related field, or equivalent experience is required.

• A minimum of 4 years of experience in care management, case management, clinical support, patient services, or a related field is essential.

• Certification in case management, care coordination, clinical specialty, or a comparable qualification is preferred.
