Nurse Care Manager, Spanish-Speaking

This is a fully remote position, open to applicants in United States.

📋 Description

• Oversee and analyze patient vitals, assessments, and alerts within the Welby Health platform.

• Offer prompt, evidence-based recommendations and escalate any clinically significant findings.

• Accurately document patient interactions in line with internal protocols and regulatory guidelines.

• Create, implement, and modify personalized care plans that address both clinical and social needs.

• Collaborate with physicians, specialists, and community resources to bridge gaps in care.

• Act as a liaison between patients and providers to enhance engagement and adherence.

• Provide patient education and coaching through telephone and secure messaging channels.

• Empower patients to effectively manage chronic conditions and develop sustainable health habits.

• Identify obstacles to care and resolve them with practical, patient-centered solutions.

• Work alongside physicians and cross-functional teams to ensure timely interventions, smooth care coordination, and an exceptional patient experience.

• Represent Welby Health’s technology-driven healthcare model to patients and their families.

⛳️ Requirements

• Bachelor's degree in nursing or a related healthcare field.

• Active and valid registered nurse (RN) license in good standing in the state of Michigan.

• Proficiency in both verbal and written Spanish.

• Approximately two (2) to five (5) years of direct patient care experience in medical-surgical or home care settings.

• Outstanding written and verbal communication abilities.

• Capability to work independently in a remote, technology-driven environment.

• Strong organizational skills with a keen attention to detail and follow-through.

• Active and valid registered nurse (RN) license in the state of Illinois or Georgia is preferred.

• Experience in case management, chronic care management, and/or telehealth is preferred.

• Familiarity with remote physiological monitoring, electronic medical record (EMR) or electronic health record (EHR) systems, and patient engagement tools is preferred.
