This is a fully remote position, open to applicants in California.
• Deliver Medical Case Management services both in-person and via phone for injured workers.
• Collaborate closely with injured workers to support their recovery journey.
• Partner with patients, families, healthcare providers, employers, referral sources, attorneys, clients, and colleagues.
• Utilize medical and nursing expertise to discuss existing and alternative treatment plans with physicians.
• Assess, strategize, implement, and evaluate patient progress effectively.
• Review treatment plans for their suitability, medical necessity, and cost efficiency.
• Participate in visits with doctors, healthcare providers, and attorneys.
• Attend discharge planning meetings at hospitals and long-term care facilities.
• Assess the appropriateness of care and devise effective long-term care strategies.
• Conduct home visits for initial patient evaluations.
• Coordinate the delivery of durable medical equipment and nursing services.
• Ensure adherence to safety protocols and regulations as outlined in the Injury and Illness Prevention Program.
• Carry out additional responsibilities as needed.
• Must hold a Registered Nurse (RN) designation.
• Graduate from an accredited nursing program.
• Current RN Licensure in the relevant state of practice.
• Possess a valid driver’s license.
• Have reliable transportation.
• Capability to travel to assigned locations as necessary.
• Strong communication and multitasking abilities in a high-volume, fast-paced, team-oriented setting.
• Capacity to meet with patients, healthcare providers, attorneys, clients, and colleagues effectively.
• Excellent interpersonal, time management, and organizational skills.
• Proficiency in computer use and technical skills.
• Competence in Microsoft Office, including Excel.
• Ability to work autonomously as well as collaboratively within a team.
• Experience as an RN Medical Case Manager is highly desirable.
• A clinical background in orthopedics, neurology, or rehabilitation is preferred.
• Background in cost containment, such as utilization review or managed care, is advantageous.
• Certification as a CCM, CIRS, or any relevant Case Management certification is preferred.
• Willingness to travel locally up to approximately 60% of the work week/month.
• Availability to work Monday through Friday during standard business hours.
• Ability to work remotely and while traveling.
• Readiness to undertake overnight travel if necessary.
