Maternity Nurse Care Manager

This is a fully remote position, open to applicants in United States.

📋 Description

• Provide coordinated, patient-focused virtual Care Management through telephone and/or messaging to enhance members’ health outcomes.

• Develop meaningful care plans in collaboration with members and the multidisciplinary care team, assisting members in reaching their objectives.

• Aid members in navigating complex medical issues, treatment options, benefits, and the healthcare system.

• Collaborate with members’ local healthcare providers to ensure cohesive care.

• Offer compassionate, ongoing follow-up care and foster supportive relationships.

• Assist members throughout routine and high-risk pregnancy experiences, from preconception to postpartum.

• Engage directly with members, addressing their concerns, responding to inquiries, and acting as a trusted advocate.

• Support fertility planning, prenatal care, delivery preparation, and postpartum recovery.

• Guide members in understanding health benefits and connecting them with suitable resources.

• Collaborate closely with providers, care coordinators, and support staff to deliver integrated remote care.

⛳️ Requirements

• Bachelor of Science in Nursing (BSN)

• Must reside in a compact NLC state.

• Active Compact RN license (in state of residence) in good standing with the nursing board of their state.

• Additional active Licensure a plus (IL, CA, MN, NY).

• CCM Certification preferred.

• 5+ years of nursing experience.

• 2+ years of experience in care, case, and disease management, ideally in a health plan, health navigator, or third-party administrator (TPA) setting.

• 2+ years of experience in labor and delivery.

• Comfort discussing a broad range of medical conditions.

• Preferred experience working remotely; must be comfortable using technology.

• Must possess high empathy.

• Ability to work efficiently and meet volume goals while maintaining quality.

• Good judgment in balancing priorities.

• Flexibility and comfort in a rapidly changing environment.

• Strict adherence to security and HIPAA regulations.

• Must be pleasant, responsive, and eager to collaborate and learn from the team.

• Excellent verbal and written communication skills.

• Ability to collect clinical histories, address questions at a patient level, and summarize findings succinctly.

• Strong proficiency in using multiple computer/medical record systems.

• Effective collaboration across multidisciplinary teams.

• Awareness of cultural and socioeconomic factors affecting members and ability to coordinate available resources.

• Exceptional grammar, attention to detail, and ability to convey medical information in clear, patient-friendly language.

• Consistent and punctual attendance at the assigned work location.

• Capability to remain seated in a stationary position for extended periods.

• Good eye-hand coordination and manual dexterity for operating a keyboard, computer, and other office-related equipment.

• Occasional exertion of up to 20 lbs of force may be necessary.

• Ability to interact appropriately with leadership, employees, and members.
