This is a fully remote position, open to applicants in New Jersey, +1 more state.
• Oversee the daily operations of the Ambulatory Population Health team, including effective workload distribution and monitoring of staff productivity.
• Collaborate on the development, execution, and assessment of ambulatory quality programs across all practices within Capital Health Medical Group.
• Standardize workflows related to population health, focusing on risk stratification, care management, transitions of care, and closure of quality measure gaps.
• Audit clinical data and health reports sourced from electronic medical records (EMR) and payer systems to identify patients for care management enrollment.
• Track team performance metrics in relation to external program requirements, HEDIS measures, and internal organizational targets.
• Facilitate patient care delivery across primary care, specialty practices, and community resources to address social determinants of health (SDOH).
• Collaborate with population health vendors to optimize care management systems, ensuring comprehensive and consistent documentation for accurate compliance reporting.
• Manage the talent lifecycle for the team, which includes interviewing, selection, onboarding, training, performance evaluations, and ongoing professional development.
• Create operational reference tools, training materials, and structured process improvement initiatives to enhance team performance.
• Assist in departmental budget management, program planning, and strategic allocation of resources.
• Carry out additional duties as assigned.
• Bachelor’s Degree in Nursing, Social Work, Public Health, or a related healthcare field.
• Valid Registered Nurse (RN) or Social Worker (LSW/LCSW) license in New Jersey and Pennsylvania, or currently pursuing PA licensure.
• AHA BLS - Healthcare Provider certification.
• Minimum of four years of clinical experience in acute care, ambulatory/outpatient settings, sub-acute rehabilitation, care management, managed care health plans, or similar environments.
• Proven knowledge of ACO models, MIPS compliance, HEDIS measures, and structured care coordination workflows.
• Strong understanding of workflow design with the ability to translate program objectives into operational workflows effectively.
• Familiarity with value-based care models, quality measures, care management, and transitional care.
• Practical skills in data analysis, including interpreting and extracting health reports.
• Proficient computer skills, including the ability to navigate various electronic medical documentation systems.
• Excellent verbal and written communication abilities, capable of conveying complex information clearly to a diverse audience.
• Demonstrated capability to autonomously direct daily work while fostering collaboration with peers and leadership.
• Proven ability to adapt quickly, manage multiple workflows simultaneously, and maintain effectiveness in a fast-paced, high-volume environment.
• Natural aptitude for building collaborative relationships across diverse teams while influencing, guiding, and motivating staff.
• Master’s degree in Nursing, Social Work, Public Health, or a related healthcare field (preferred).
• Certification in Case Management, Ambulatory Care, Care Coordination, or another relevant professional area (preferred).
• Two years of supervisory, team lead, or program management experience (preferred).
• Direct experience in a Population Health setting (preferred).
