This is a fully remote position, open to applicants in United States.
• Oversee Operation Integration Specialists (OIS) involved in administrative and clinical practice transformation programs as well as alternative payment models.
• Instruct the OIS team regarding the elements, measures, tasks, and tools that facilitate practice transformation functions and activities.
• Provide leadership, motivation, and direction to ensure projects progress towards successful completion.
• Maintain a comprehensive understanding of care models, data elements, tools, and resources.
• Act as a liaison between the OIS team and population health teams, monitoring, and addressing issues, challenges, barriers, and successes.
• Oversee OIS team activities, including rounding schedules, communications, issue resolution, provider onboarding processes, and compliance with government reporting requirements.
• Create and sustain team plans that encompass resources, tasks, timelines, program requirements, and government deadlines.
• Assist providers and care teams in executing process and quality improvement activities and workflows.
• Serve as the main resource for providers regarding alternative payment practice transformation programs through education and communication.
• Manage the collection, analysis, translation, and reporting of quality and performance metrics.
• Collaborate with data analytics and EPIC EHR teams to gather actionable insights for enhancing patient care, population health, and cost management.
• Oversee the collection, tracking, analysis, and reporting of practice demographics and transformation activities.
• Participate in mandatory training, education, and learning events.
• Act as a subject matter expert on government alternative payment models.
• Execute additional duties as assigned.
• Bachelor's degree in Healthcare Administration or a nursing-related field.
• Preferred: Master's degree in Healthcare Administration or a nursing-related field.
• Minimum of 5 years of experience in an ambulatory healthcare setting, ideally in a leadership capacity within a highly matrixed organization.
• Expertise in physician practice operations and practice transformation activities.
• Strong analytical capabilities.
• Ability to manage multiple projects simultaneously.
• Capacity to work independently and proactively in a high-pressure medical environment.
• Detail-oriented with a focus on accuracy.
• Skill in managing time, priorities, and resources effectively.
• Excellent verbal, written, and interpersonal communication skills, with the ability to adjust communication style to engage the target audience.
• Competence in Microsoft Office applications such as Word, Excel, and PowerPoint.
• Willingness to travel locally and regionally.
• Ability to plan, conduct, and facilitate meetings and networking events.
• Familiarity with EHRs, preferably EPIC.
• Knowledge of process improvement models such as PDSA, A3, CQI, Lean, and Six Sigma.
• Willingness to travel up to 10% annually.
