This is a fully remote position, open to applicants in United States.

📋 Description

• Assist Practice Liaisons during health center presentations by addressing comprehensive clinical and billing inquiries.

• Take charge of the onboarding process for newly contracted health centers alongside Practice Liaisons, converting the fee-for-service model into actionable startup procedures.

• Act as the tactical subject matter expert on the delivery of fee-for-service care management.

• Address health center billing challenges by identifying root causes in documentation or workflow processes.

• Develop and maintain FAQs and training resources.

• Expand expertise across the fee-for-service portfolio by training other trainers.

⛳️ Requirements

• Active Registered Nurse (RN) license in good standing, or equivalent clinical licensure.

• 5 to 10 years of experience in fee-for-service chronic care management, transitions of care, or advanced primary care, including practical experience across the entire workflow: care plan development, medication reconciliation, EMR documentation, and billing.

• Experience in working with or supporting Federally Qualified Health Centers (FQHCs) or similar community-oriented healthcare environments.

• Proven ability to trace billing discrepancies back to the documentation or procedural steps that led to them.

• Comfortable addressing detailed, state-specific, and payer-specific questions from health center personnel.

• Certification in Case Management (CCM), Advanced Professional Case Management (APCM), Transitions of Care (ToC), and Collaborative Care Management (CoCM).

• Experience in training or onboarding new staff or partner organizations on clinical workflows.

• Knowledge of value-based care models and population health strategies.
