This is a fully remote position, open to applicants in Arizona, +9 more states.
• Oversee clinical management and guidance of Enhanced Care Management (ECM) personnel addressing the medical, behavioral health, and social determinants of health for clients.
• Engage with ECM staff and, when necessary, patients through telephone, telehealth, or Interdisciplinary Care Team meetings.
• Review medication management and adherence, ensuring necessary follow-ups with primary care, behavioral health, and specialist providers.
• Assist ECM staff in navigating crises.
• Collaborate with the clients’ comprehensive care teams to advocate for their needs and secure necessary services.
• Evaluate progress towards personalized, patient-centered care plan objectives.
• Monitor team performance, provide coaching, and manage staff outcomes.
• Supervise around 10–15 direct reports.
• Manage client outreach, enrollment processes, and Individual Care Plan development.
• Determine if client needs can be effectively handled by an ECM Lead Care Manager and act as an escalation point.
• Propel the team to achieve Key Performance Indicators (KPIs).
• Work in conjunction with Enhanced Care Management Directors to implement changes and improve processes.
• Offer clinical oversight approximately 30% of the time, including supervision of clinical staff, clinical sign-offs, oversight of care plans, Interdisciplinary Team meetings, and consultation on complex cases.
• Provide direct care management as needed.
• Travel up to 20–25% for training, meetings, and other business-related activities.
• Must reside in Arizona, California, Colorado, Florida, Georgia, Illinois, Nevada, North Carolina, Oregon, Texas, or Washington.
• Available to work Monday through Friday, from 8:30 am to 5:00 pm Pacific Time, with flexibility for potential evening and weekend shifts.
• Possess an active, unrestricted RN license, with the ability to secure California licensure within 6 months of hire.
• Willingness to obtain any additional state licensure as required.
• Minimum of 2 years of experience in people management; 4 years is preferred.
• At least 2–3 years of care management experience is required.
• Minimum of 1–2 years of experience in post-acute care.
• Demonstrated leadership and change management success in rapidly growing environments.
• Strong organizational skills and the ability to work independently.
• Proficient in Microsoft and Google applications; experience with Salesforce is advantageous.
• Excellent communication and motivational abilities, including skills for conflict resolution.
• Familiarity with Lean Six Sigma methodology is preferred.
• Must have legal authorization to work in the United States.
• Completion of background screening prior to employment is required.
