This is a fully remote position, open to applicants in Arizona, +9 more states.
β’ Oversee and manage the clinical operations of ECM staff, addressing the medical, behavioral health, and social determinant needs of clients.
β’ Engage in communication with ECM staff and, on occasion, patients via telephone, telehealth, or during Interdisciplinary Care Team meetings.
β’ Discuss medication management and adherence, ensuring appropriate follow-up with primary care, behavioral health, and specialist providers.
β’ Provide guidance during crises faced by ECM staff.
β’ Collaborate with the wider care team to advocate for clients and guarantee they receive necessary services.
β’ Evaluate progress towards individual care plan goals centered around the patient.
β’ Monitor team advancement, coach staff, and manage performance as necessary.
β’ Supervise around 10β15 direct reports within the ECM team.
β’ Oversee client outreach, enrollments, and the development of Individual Care Plans.
β’ Determine if client needs can be addressed by an ECM Lead Care Manager and act as an escalation point when required.
β’ Drive the team to achieve key performance indicators.
β’ Work in conjunction with Enhanced Care Management Directors on process improvements and changes.
β’ Provide clinical oversight for approximately 30% of the time, which includes supervising clinical staff, conducting clinical sign-offs, overseeing care plans, participating in IDT meetings, and consulting on complex cases.
β’ Offer direct care management as necessary.
β’ Travel up to 20β25% for training, meetings, and other business-related needs.
β’ Must reside in Arizona, California, Colorado, Florida, Georgia, Illinois, Nevada, North Carolina, Oregon, Texas, or Washington.
β’ Available to work Monday through Friday, from 8:30am to 5:00pm Pacific Time, with flexibility for potential evening and weekend shifts.
β’ An active, unrestricted LCSW or LMFT license in California is required.
β’ Willingness to obtain additional state licensure as necessary.
β’ Minimum of 2 years in people management; 4 years preferred.
β’ 2β3 years of experience in care management.
β’ 1β2 years of experience in post-acute care.
β’ Demonstrated, results-oriented leadership experience in change management within rapidly growing environments.
β’ Strong organizational skills and the ability to work independently.
β’ Proficient in Microsoft and Google applications; experience with Salesforce is advantageous.
β’ Exceptional communication and motivational skills, including techniques for conflict resolution.
β’ Working knowledge of Lean Six Sigma methodology is preferred.
β’ Legally authorized to work in the United States.
β’ Completion of required background screening is mandatory before employment.
