Manager – Care Coaching

This is a fully remote position, open to applicants in Wisconsin.

📋 Description

• Guide and assist Field Nurses and Care Coaches/IDT to ensure Family Care benefits align with the DHS contract.

• Identify educational needs and organize mentorship, orientation, and continuous learning for IDT personnel.

• Evaluate and mentor staff on IDT competencies and performance expectations.

• Analyze reports, evaluate processes, and review documentation.

• Assess utilization and the quality of care management for the members served.

• Collaborate with various departments to guarantee the quality and effectiveness of provider services.

• Educate community partners.

• Engage in the hiring process by selecting, orienting, and training new associates.

• Lead in-person unit meetings and participate in quality visits.

• Provide daily leadership and mentorship to staff.

• Report directly to the Associate Director of Care Coaching.

• Supervise approximately 11–15 direct reports.

• Occasionally travel to Humana offices for training or meetings.

⛳️ Requirements

• Bachelor's degree in human services or a related area, or an associate degree in nursing.

• At least 1 year of leadership experience in the human services sector.

• A minimum of 3 years of experience in the Family Care program.

• Preferred: 5 years of experience working with individuals with intellectual/developmental disabilities, physical disabilities, or elderly individuals.

• Preferred: Reside within a 2-hour commute of Dane County.

• Preferred: Master's degree in public administration, social work, or a related discipline.

• Ability to work Monday–Friday, from 8:00 am to 4:30 pm CST.

• Willingness to travel at least once a month, with potential for additional travel.

• Minimum home internet speed of 25 Mbps for download and 10 Mbps for upload.

• Capability to work from a dedicated area without ongoing interruptions to safeguard member PHI/HIPAA information.
