Manager, Care Coaching

This is a fully remote position, open to applicants in Wisconsin.

📋 Description

• Assist the Care Management team in their operations.

• Provide daily guidance and mentorship to team members.

• Guide and support Field Nurses and Care Coaches/IDT to ensure compliance with Family Care benefits under the DHS contract.

• Identify educational needs and arrange for mentorship opportunities.

• Establish expectations and facilitate orientation and continuous learning for IDT personnel.

• Evaluate and coach staff on primary competencies and performance benchmarks for IDT.

• Review reports, assess procedures, and evaluate documentation.

• Evaluate utilization and quality of care management for each member served.

• Collaborate with various departments to guarantee the quality and effectiveness of provider services.

• Educate community partners about services.

• Participate in the hiring process by selecting, orienting, and training new associates.

• Lead in-person unit meetings and attend quality visits as necessary.

• Travel at least once a month, with the possibility of additional trips.

• Report directly to the Associate Director of Care Coaching.

• Supervise approximately 11–15 direct reports.

⛳️ Requirements

• Bachelor's degree in human services or a related field, or an associate degree in nursing.

• At least 1 year of leadership experience in the human services sector.

• A minimum of 3 years of experience in the Family Care program.

• Must reside within 1 hour of either Black River Falls or Blair, WI.

• Preferred: 5 years of experience working with individuals with intellectual/developmental disabilities, physical disabilities, or elderly individuals.

• Minimum home internet speed of 25 Mbps for download and 10 Mbps for upload.

• Must work from a designated space free from ongoing interruptions to protect member PHI/HIPAA information.
