Manager, Advanced Practice Provider – Transitions of Care

This is a fully remote position, open to applicants in United States.

📋 Description

• Oversee clinical and operational aspects of APPs within the Transitions of Care program while continuing to deliver clinical services.

• Collaborate with the TOC management team to enhance a culture of teamwork, empower staff on the front lines, and pinpoint opportunities for improving patient care.

• Dedicate approximately 40% of your time to providing patient care within the TOC.

• Deliver exceptional care to patients throughout the 30-day post-discharge period, identify care gaps, escalate cases involving symptomatic patients, and work to minimize preventable readmissions.

• Act as the APP manager and a member of the TOC leadership team for around 60% of your time.

• Ensure accountability and oversight for the clinical performance of the TOC APP organization.

• Manage the performance of frontline TOC APPs to meet required targets.

• Develop, monitor, and manage clinical and operational Key Performance Indicators (KPIs).

• Identify areas for improvement and implement strategies to meet targeted metrics.

• Lead, empower, inspire, and align the TOC APP team, including conducting bi-weekly one-on-one meetings with direct reports.

• Establish personalized goals and evaluate individual performance.

• Work in conjunction with the program medical director and APP managers to enhance team processes and workflows.

• Assist in the recruitment strategy, hiring, and onboarding of new TOC APPs.

• Create clinical processes and policies.

• Monitor the allocation of clinical resources and adjust them to fulfill visit requirements.

• Manage schedules, time-off requests, schedule density, and any patient-impacting schedule changes.

• Provide onboarding support for new providers in collaboration with the education and training team.

• Serve as a liaison to TOC leadership regarding the status, issues, and opportunities related to the APP organization.

⛳️ Requirements

• A minimum of 5 years of experience in clinical practice as an Advanced Practice Provider (NP/PA).

• At least 1 year of experience working with post-discharge patients as an Advanced Practice Provider (NP/PA).

• Over 2 years of prior management or team leadership experience.

• A valid and clear RN and APRN license or Physician Assistant license in your home state.

• National APRN certification and/or the ability to obtain licensure in multiple states is required at the time of hire and must be maintained during employment.

• Availability to work Monday through Friday, from 9:30 AM to 6:00 PM EST.

• Experience conducting visits via telehealth video platforms.

• Proven experience managing post-transition patients and recognizing the need for immediate interventions.

• Background in managing acute and chronic disease exacerbations, including CHF, diabetic emergencies, COPD exacerbations, and hypertensive crises.

• Experience collaborating on process improvement initiatives with colleagues outside of your immediate role.

• A willingness to adapt and grow using PDSA principles.
