This is a fully remote position, open to applicants in North Carolina.
• Develop, evaluate, and coordinate comprehensive care management initiatives aimed at achieving quality and cost-effective healthcare outcomes.
• Create or assist in the formulation of individualized service care plans for long-term care members.
• Provide education to members and their families/caregivers regarding available services, benefits, procedures, provider instructions, referrals, and healthcare entitlements.
• Assess member needs and available resources, recommending or facilitating plans to achieve optimal outcomes.
• Identify necessary providers, specialists, and community resources for long-term care.
• Coordinate member and/or family/caregiver interactions with care provider teams to ensure accessibility of services.
• Offer resource support for employment, housing, participant direction, independent living, justice, foster care, and other essential needs.
• Monitor care plans, member status, and outcomes, suggesting updates based on evolving member needs.
• Engage with long-term care healthcare providers and partners.
• Collect, document, and maintain member information and care management activities to ensure regulatory compliance.
• Conduct home and/or other site visits as required.
• Provide feedback to leadership concerning quality of care and service delivery enhancements.
• Perform additional duties as assigned while adhering to all policies and standards.
• Must reside in Iredell, Rowan, Alexander, Davie, Davidson, Yadkin, Catawba, Wilkes, Lincoln, or Stanley County, North Carolina.
• RN or LCSW licensure is required.
• A Bachelor's degree is mandatory, with specified qualifying-experience alternatives.
• 2–4 years of relevant experience is necessary.
• For North Carolina Standard Plan: two years of prior LTSS and/or HCBS coordination, care delivery monitoring, and care management experience is required.
• Previous experience in social work, geriatrics, gerontology, pediatrics, or human services is preferred.
• Experience in interviewing and assessing member needs is essential.
• Knowledge and experience in caseload management and casework practices are required.
• Familiarity with determining eligibility for DHSS programs is necessary.
• Understanding of Federal and State regulations as they pertain to DHSS programs is essential.
• Ability to effectively troubleshoot and locate community resources is crucial.
• Capacity to collaborate with caregivers, involved State agency representatives, and providers is important.
• Strong interpersonal skills are necessary.
• Ability to practice Cultural Competency, with an awareness and respect for diversity.
• Knowledge of the needs and service delivery systems for all populations within the case manager’s caseload is required.
• Technological proficiency is expected.
• Exceptional communication and customer service skills are essential.
• Strong assessment skills are required.
• Change management capabilities are necessary.
